appropriate section headings. · Reference sources must be cited in the text of the report and
listed appropriately at the end in a reference list using Harvard referencing

Late submission |

2. 2.1 The investigation authority has conducted the investigation. After the completion the investigation, the accident investigation authority needs to give some conclusions and recommendations. In your opinion, was everything covered in the report? Did you discover any issues that were not mentioned in the report? What was the reason these issues were not mentioned in the report? (LO3, total points 15)
3. If you had to design polices, give suggestions and recommendations in order to help reduce the human factors that could result into causing future potential accidents, what would those (policies, suggestions recommendations be). What suggestions for changes would you recommend in order to reduce the effects of human factors on active failures? (LO5, total points 25)
