Jane Holland is a 40-year-old lady who attends your clinic complaining of discomfort and occasional pain from her callus which has worsened over the past year. She informs you she has a long history of problems with both of her big toenails which includes previous intervention by podiatry where she underwent partial nail avulsions to the left medial only.

She notices her symptoms particularly after prolonged activities and are eased with resting.

Social History

Jane works as a paramedic frequently completing 12 hours plus shifts. She wears standard issue safety shoes for work, with other styles of footwear including ballet pumps and fashion trainers when not at work and 3-inch heels for dress occasions. She walks a few miles daily due to dog walking and notices her symptoms towards the end of the walk.

Clinical observations

Both feet appear to have a low medial arch profile. Both 1st MTP joints appear normal in size with no signs of erythema or irritations from footwear. Both 15 IP joints are hyperextended with callus present to the medial aspect of both joints. Both 1st nails appear involuted in shape with some evidence of previous erythema to both right sulci. Her ballet pumps and trainers show signs of internal wear around the toe box and are slightly warped as a result.
