The important clues are his heart rate of 126, blood pressure of 105/60, weak radial pulse, pale clammy appearance, pelvic and abdominal tenderness, chest crepitus and open tibia/fibula fracture.

The challenge is to decide which injuries are immediately life-threatening and explain this using the patient`s physiology rather than simply listing possible diagnoses.

Key concerns include:

concealed bleeding from the pelvis or abdomen

significant right-sided chest trauma

blood loss from the open leg fracture

developing traumatic shock despite a systolic blood pressure above 90 mmHg

The second part of the assignment then requires you to choose one management intervention, such as TXA, blood products, chest injury management, pelvic management or analgesia—and critically analyse why it is appropriate for this patient.

Assignment - Case Study Case â–¶ You are a critical care clinician, responding to an emergency trauma patient.

â–¶ A 32-year-old male has been involved in a road traffic collision. He is a motorcyclist, who was travelling at approx. 40mph (60kmh) when a car pulled out from a junction, and he impacted the passenger side door. He was thrown over the bonnet of the car and landed on the road surface.

â–¶ On arrival of the emergency services, he was lying on his back, a police officer had removed his helmet and was holding his head in-line. He is of average height and build, weighing approx. 80kg.

â–¶ The Paramedic first responder has removed his bike leathers and performed a primary survey. She found his airway is patent, and he responds to voice. She notes he has a respiratory rate of 34 and crepitus to the right side of his chest with significant pain when palpated and oxygen saturations of 98% on a non-rebreather mask (96% on air).

â–¶ He has a weak radial pulse, appears pale and clammy and is tender over the ASIS of his pelvis and lower abdomen. He is tachycardic at 126 beats per minute with a blood pressure of 105/60.

â–¶ Both arms appear intact, and he can move them, his right leg appears to have an open tibia/fibula fracture and his left leg looks intact; he can move his toes on request on both feet.

â–¶ He has no sign of a head injury, and there are some scratch marks to the helmet from the incident, but it remains structurally intact. He has no apparent head injury, and pupils are size 4 bilaterally reactive. He is in a lot of pain, specifically to his chest and right leg when asked. His GCS is 14 (E3:V5:M6).
