NP 2.1g: Engage in shared decision-making with consideration of determinants of health. (Use this I had a newly diagnosis diabetic patient who reports not working at this time. It didn’t know how he would be able to buy food to follow his diabetes diet. I helped the SW find community centers and churches in his neighborhood that did food drives and we had him meet with the nutritionist to go over what food are good for diabetic patients.
