Assignment brief This assignment involves writing a comprehensive case study based on one of the five conditions provided. It will involve selecting a patient you have looked after in practice with one of the provided conditions and focusing on the complexity of the condition and the required nursing care. It will include an outline of the patient’s condition, covering the pathophysiology, signs, symptoms, and its impact on the patient. A detailed nursing assessment will be conducted following a recognised holistic framework, with a care plan developed in collaboration with multidisciplinary professionals. The pharmaceutical management will be evaluated, considering polypharmacy, medication safety, and family/patient education.

Additionally, the assignment will explore the patient’s holistic care needs, including the impact on their quality of life, mental health, and daily functioning, and how factors like social support, culture, and health literacy influence care. The nurse’s leadership role in coordinating care, ensuring patient safety, and adhering to ethical and legal responsibilities will also be discussed, with reference to professional standards like the NMC Code.
