Part 1: Project Report This first part of your assessment is a final report that encapsulates your entire project. The deliverable for your final report will be a 10- to 12-minute video of you presenting your project and its outcomes. You will create a Microsoft® PowerPoint® presentation to support your report. As you prepare your report, keep in mind that regardless of who the audience is, the report should clearly convey who was involved in the project and at what level, what was done, how it was done, and why the project was needed (Sipes, 2019). Address the following areas in your presentation: Introduction Organization Background information Objectives (Healthy People 2030 objective) Intended improvement (SMART goal) Method Project setting Planned interventions Implementation Analysis and evaluation methods Measures Analysis Results Outcomes analysis Summary of outcomes Discussion Lessons learned Interpretation of findings Recommendations Conclusion References (This should be on its own slide.) Feel free to incorporate charts, tables, or graphs depicting any significant data collected into the report if the resource provides a critical visual element that supports your final evaluation. Any charts, tables, or graphs depicting data that is not directly discussed, or is only referenced once in your report, should be included as an appendix or on the References slide. Format all citations and references according to APA guidelines. Record your presentation using Microsoft Stream or other recording software. You will submit the link to your video as the deliverable for Part 1. Part 2: Dissemination, Sustainability, and Advocacy For the master’s-prepared nurse informaticist, the final step of project development is dissemination of the results of your project. This step is just as important as the project itself; no one benefits from findings that are not disseminated. Through dissemination of your project outcomes, you can translate evidence into practice and lead change to improve quality care outcomes. Even projects that were not successful in achieving their goals should be disseminated so that financial resources are not wasted repeating the same project. After leading a project that produces transferable knowledge, it is the nurse informaticist’s responsibility to disseminate that information for the betterment of all. Dissemination Plan Develop a plan for disseminating the results of your project beyond the primary project stakeholders. Your plan must address the following: Why (the purpose of dissemination) What: will be disseminated (the message) To whom (the audience) How (the method) When (the timing) Sustainability Plan Effective dissemination is essential for sustainability. According to the KTA framework, sustainability looks like this in the health care setting: Develop a sustainability plan that will ensure the factors above will be in place. Sustainability will depend upon: Identifying strong program champions to rally around the improvement. Choosing a focus that fits with the organizational mission. Continually evaluating process and outcome measures. Advocacy Advocacy is a fundamental component of nursing. How can you continue to advocate for the patient population or for the process change that you focused on in your project? Can you incorporate advocacy into your dissemination and sustainability plans? Summarize in 525 to 700 words your dissemination and sustainability plans and discuss how you will integrate advocacy. Include at least 5 references and format according to APA guidelines.
