NOTE: be as detailed as possible! i. e. patient appears to have good hygiene "as evidenced by presenting in clean and appropriate clothing and does not emit body odor"; patient is calm and relaxed "as evidenced by..."; DO NOT USE any abbreviations such as "A/O x'3"... that is NOT acceptable. Please use full sentences and "as evidenced by" in your findings.
